Post-hospital home support

Coming home is easier with help ready.

The days after a hospital discharge are demanding for the whole family. Our caregivers provide practical, non-clinical support at home — routine, mobility, meals, personal care, medication reminders and family coordination — according to the approved care scope and existing discharge instructions.

This supports daily living during recovery. It does not replace medical treatment or clinical follow-up. When clinical help is needed, we can connect you with doctor visits, nurse visits or physiotherapy, quoted separately.

Planning a discharge? Contact us early so support can be in place before your loved one returns home.

Non-clinical support. Caregivers do not diagnose, treat, prescribe or administer medication beyond reminders, or provide emergency response. Doctor and nurse visits, physiotherapy and other clinical services can be arranged separately — see pricing.

Let’s talk about the right care for your loved one.

Whether you need a few hours of help, overnight support or a long-term caregiver, a coordinator will guide you — with no obligation until you confirm your written quotation.
